If an employee has to be out for 12 weeks of FMLA leave, can the supervisor rate the employee lower on the annual performance appraisal? Like if you had to do 100 assignments to get 5 out of 5, but the employee only did 75 assignments that rating year, can the supervisor say the employee's performance was only say 3 out 5?

Probably but that's not a reflection in a negative sense.
Let's say 100 assignments would get you a 5 for exceeding expectations and 80 assignments would get you be a meeting the expectations. By not being at work for 12 weeks, your performance is reduced but so are your expectations. However that doesn't necessarily translate into a lower requirement for exceeding expectation because the meeting expectations is lowered temporarily because of FMLA.

I asked my Supervisor who copied the previous years PAR and gave me a lower rating if the PAR was supposed to be Objective, or Subjective. What he said next blew me away~
