I noticed on a couple announcements is said submit a SF-15 along with the proof. I've never done this. I have my documents saved in Application Manager and one of them is just my VA letter I got that says my percentage. Am I supposed to submit this SF-15 to qualify for 10-pt preference and if so do I just fill it out then submit it each time?

If you want to receive the 10-pt. consideration, you need to submit the documentation.  Sometimes they allow you to do (prove) it if/when you are selected.

I submit my VA Letter that shows my percentage I just never added this SF-15.

If the vacancy announcement says that an SF-15 is required, then you have to provide one in order to receive 5- or 10-pt preference.  If you don't, you may get filtered out quickly because you don't have all of the required documents.  You just have to read carefully "how to apply".  I made those mistakes early on but have learned.  I know your VA documents are proof but some organizations also ask for the SF-15 too.

The SF-15 is specific for each job annoucement.  You are able to apply to Public and Status jobs based on your 10-pt preference.  Which means you would have to complete two SF-15s.  One for the Public and one for the Status Candidates.

So do you just type in the information on the form and scan it and attach it as a document like all your other documents? I see it has a block for the AO to sign and date. When do they do that?

Yes, I just type in the info, print it, sign it, scan it and attach it like my other documents.  The AO part is completed by the agency and not your responsibility.  Also don't forget there are 2-pages on the SF-15.  I fill out the back to the best of my knowledge and so far have recieved the 10-pt preference each time.  So I must be doing something right.

Do you have to put the job announcement number on the job you're applying for into block #2 and the date of application in block #4?

Yes. You should fill out the form completely, except where it says "For Use By Apointing Officer Only".
