  My Dr. wants me to have a shoe implant for my arch but we dont know how to submit it to owcp for approval, anyone know how this is done?

Your doctor would submit the request through ACS. All physician's dealing with OWCP Claimant's must be registered with ACS, so your physician should already be registered.
I would also suggest your physician write something on his/her letterhead that states you need the shoe implant and WHY you need it. The physician should relate the shoe implant to the accepted conditions.

  I found out that the Dr. has to submit a durable medical equipment form and it gets aproved that way.
