How does one go about adding another injury to an accepted claim?  Example.  If your claim is approved for Carpal Tunnel and thru additional testing MRI, etc. your doctor discovers a Nerve impingement in the Neck area how would you go about adding that to your claim. 
Do you have to file all new paperwork,  the CA2 etc?

If it's a new condition that happens during one day, one shift that's a CA-1
If it's a condition that happened over time it's a CA-2
If the CTS caused the nerve impingement that's a consequential injury. It happened as a result of or progression of the CTS. There is no form for a consequential injury. You need a medical narrative explaining the connection and you would need to request the consequential injury be added.
