My email asked to complete an applicant disclosure and drug questionnaire form. Except I only received the drug questionnaire as an attachment. Replied to the email for clarification.

Following this thread for some time now.

I just got my packet too! Aug 18 applicant and I'm concerned...
If I tried recreational marijuana once in 2015 because I was dealing with alot of emotional and physical pain, should I list this? I have never used to again since because I am seeing therapists and stuff now.
I want to be extremely honest but don't want to disqualify myself as well. ...

I’m in a similar position but according to DEA policy, limited marijuana usage is okay as long as it’s not within three years.

Looks like we got movement finally. 8/18 applicant here -

Did y'all just get only the Drug Disclosure and Questionnaire (2 forms to complete)?

I just moved and am awaiting a mailing address, is it likely we will get a packet or some sort? I really don't want to miss anything via mail if that's the case... My address is all sorts of goofed up because the apartment we signed for had pushed out our move-in date by 2 months, so we are temporarily housing with family and in the process of obtaining a new lease elsewhere while trying to terminate the one that screwed our original move-in...

Good luck to all! And congrats !!!!!

Thank you, guess I'll have to inform the rep that sent me the packet in case.

I only received a 2 page drug questionnaire. There was also a typo in the email message: “return the forms to me by COB Thursday September 25, 2019.”

Except Thursday is the 26th. Good thing I always send everything that’s requested the same day.

Hopefully you got the 2nd form when you requested clarification? My email did say to return by COB Weds 9/25 but it sounds like you were on top of it regardless. Good luck!
